Use this form to report minor (less than 200 items) or major incidents (more than 200 items) including water damage, mould, bug infestations, etc. If the incident involves multiple materials you only have to fill out one form. This form is to be filled as soon as the emergency or disaster have been mitigated. It’s important to document as much as possible in case materials where exposed to activities that might produce long term damage or unexpected results.
